What are the Signs and Symptoms?
AFS normally presents with signs and symptoms of nasal airway obstruction, allergic rhinitis, or chronic sinusitis that includes nasal congestion, purulent Nasal discharge, postnasal drainage, or headaches. Often, presentation of AFS is subtle. Patients typically complain of gradual nasal airway obstruction and production of semi-solid sticky nasal crusts . Development of nasal airway obstruction may have been so gradual that the patient is unaware of its presence. Because of is slow progression, if facial asymmetry is present, its progression often is unrecognized by the patient and family members. Pain is uncommon among patients with AFS and suggests the concomitant presence of a bacterial rhinosinusitis. Patients with AFS are allergic but generally have been unresponsive to antihistamines, intranasal corticosteroids, and prior immunotherapy. Use of systemic corticosteroids may produce some relief of symptoms, but relapse is typical following completion of therapy. In contrast to patients who suffer invasive fungal sinusitis, patients with AFS always are immunocompetent. The range of physical findings on examination typically is broad, from nasal airway obstruction resulting from nasal inflammation and polyposis to gross facial disfigurement and orbital or ocular abnormalities.

What is the treatment ?
All patients with AFS should have surgical debridement of their sinuses Endoscopic sinus surgery offers the best and most comprehensive surgical option followed meticulously by judicious Post-operative Endosopy. Oral Corticosteroids have prominent role in reducing the recurrence which may have to be given for prolonged periods . Any recurrent disease should be surgically removed as well.
