HelloMD.com: UROLOGY (URINARY AND GENETAL SYSTEMS)

PROSTRATE ENLARGEMENT

When should one suspect enlargement?

  1. Because the gland trends to obstruct the urinary passage, so the person will have a weak stream of urine and there will be difficulty in initiating the act of urination

  2. The person will have to strain a lot to urinate.

  3. Night frequency of passing the urine increases and the patient is unable to sleep properly at night.

  4. There is sense of incomplete evacuation of bladder and the patient generally looks for a place to pass his urine wherever he goes.

  5. Later on as the symptoms progress in severity, there may be complete obstruction/ retention of urine.

  6. If the patient delays treatment for a long time, he may develop kidney damage due to back pressure of incompletely passed urine.

What are the treatment modalities available ?

Medical treatment by giving various drugs may relieve the symptoms temporarily but mostly all the patients have to undergo surgery sooner or later.

How is surgery carried out?
It is carried out under local anesthesia. A cystoscope (small tube with optical illumination and a facility to introduce various instruments) is introduced through urethra and the prostrate gland obstructing the flow of urine is cut out in small pieces.

What are the usual complications?
Immediate complication is that urine which comes out in the tube can be slightly blood stained but usually it does not last for more than a day or two. Other complication is infection which if occurs, can be managed with antibiotics. Sometimes after removal of catheter (which is mostly taken out after a week or so) there may be some loss of control over the act of urination for a few days and it can be managed by teaching the patient a few exercises.
