Puerto Rico Medical Cannabis Program:
You must complete and notarize a Medical Cannabis Card application obtained from PR’s Department of Health (can be obtained on online).
Obtain a Medical Cannabis Certificate from a Medical Doctor registered with the PR DOH Medical Cannabis Program.
Submit a copy of a valid ID.
You must also, submit a $25.00 check or money order payable to "Secretario de Hacienda".

Anxiety
Arthiritis
Cancer or other conditions that cause severe nausea
Crohn’s disease
Epilepsy
Fibromyalgia
Hepatitis C
HIV/AIDS or other conditions that cause cachexia
Migraines
Multiple Sclerosis or other conditions that cause severe spasms
Parkinson’s disease
Rheumatoid Arthritis
Spinal cord injury or other conditions that cause severe pain
