Why is my card only good for three months?

In some cases, the practitioner has concerns about your medical condition and/or history that warrant approving a recommendation for less than 12 months. In three months time, you will be prompted to return for a follow-up consultation; this gives both you and the practitioner an opportunity to discuss how your treatment plan is working out.
